Essential functions: The Registered Nurse, Home Care possesses and practices appropriate physical backssment skills, including backssment of the adolescent, adult and geriatric patient, family and home situation.
Makes initial evaluation visit. Provides ongoing backssment of patient's condition and nursing needs. Prioritizes patient's problems or needs according to urgency to reflect the physical, psychological, emotional and spiritual aspects of care of the patient. Completes OASIS backssments per Medicare guidelines Fulfills on call responsibilities
when assigned. Acts as a case manager to coordinate all home care services. Supervises Home Health Aides. Clarifies and implements physician orders and nursing care plan according to established standards.
Performs medication reconciliation and instructs patient/family in medication use and precautions. Develops and revises an individualized plan of care and sets mutually acceptable goals with patient/patient's family. Documents visits in Homecare Homebase. Grand View Health's Home Care department serves Bucks and Montgomery communities. GVH Home Care strives for excellency by assisting patients with their health needs in the comfort of their home. The Home Care staff is expected
to commute within a 15-mile radius of Grand View Hospital.
