The social worker rounds on their assigned patients with the multidisciplinary team. Additionally, the social worker will participate in quality meetings with their assigned clinic. Candidate must possess communication skills, have knowledge of available community resources, document all interventions; participate in departmental meetings/in-services,

Minimum Qualifications: L. M. S. W. L. C. S. W. or eligible, 3 years relevant post-master's experience (preferably in a hospital and/or healthcare environment) required. New M. S. W. graduates who are L. M. S. W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L. M. S. W. within 12 months of receipt of limited permit. Preferred Qualifications: Transplant experience preferred and/or working knowledge and experience.
Work Days: Monday-Friday 8:30-5 with occasional on-call/holiday coverage on rotation based on operational needs.
