Screen and backss patients for common mental health and substance abuse disorders and provide clinically appropriate treatment based on an objective mental health backssment. Provide patient education about common mental health and substance abuse disorders and available treatment options. Monitor patients (in person or by telephone) for changes
in clinical symptoms and treatment side effects or complications. Support psychotropic medication management prescribed by providers focusing on treatment, adherence, side effects and other complications, and effectiveness of treatment.
Provide brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving Treatment, Motivational Interviewing, or other treatments appropriate for outpatient care settings. Provide or facilitate in-clinic or outside referrals to evidence-based psychosocial treatments (e. g. CBT, IPT) as clinically indicated. Participate in regularly scheduled (usually weekly) caseload consultation with the psychiatric consultant or CRNP-DNP
and communicate resulting treatment recommendations to the patient's primary care provider, if needed.
Facilitate patient engagement and follow-up in behavioral health services. Track patient follow-up and clinical outcomes. Document patient visits and telephone encounters with the individual patient record within the electronic medical record. Document patient progress and treatment recommendations in the registry so they can be easily shared with the psychiatrist, CRNP-DNP and other treating providers. Facilitate treatment plan changes for patients who are not improving as expected (including medication management and other treatment modalities. Facilitate referrals for clinically indicated services outside the outpatient clinic setting (e.
g. social services such as but not limited to vocational rehabilitation, mental health specialty care, substance abuse treatment). Complete timely and accurate documentation within the electronic medical record. Assist with the ongoing support of assigned telepsychiatry services. Participate in planned supervision and case consultation conferences with psychiatrist and CRNP. Required Credentials/Experience: Master's degree in social work with a minimum of two years' experience. Licensed Clinical Social Worker (LCSW) certification required.
