Secure Authorization for Release of Medical Information Conduct initial interview, collecting current and historical patient information Obtain medical guidelines from attending physician through completion of Physical Capabilities Evaluation Form Coordinate receipt of all pertinent details necessary for backssment of medical condition Determine case direction, establishing specific rehabilitation objectives and goals towards physical and psychological recovery and define associated actions Coordinate service delivery from external providers and community resources Implement medical management through routine physician contact and routine contact with other relevant medical personnel (e.
g. physical therapist, occupational therapist, nutritionist, etc. ) Coordinate and monitor participation in appropriate rehabilitative programs Monitor and evaluate rehabilitative progress and obtainment of goal Secure completed Job Description from the Employer, and pursue job accommodations as appropriate Coordinate and maintain schedule of daily activities Formulate timely case reports, to include presentation of factual information and analysis of such Determine vocational interventions and maintain regular contact with Vocational Case Manager when appropriate to monitor Consumer progress Adhere to all applicable physician, hospital, facility procedures and protocols during case management
activities; build and maintain productive relationships with all professional contacts As needed, assign any clinical duties to appropriate professional staff As appropriate, escalate issues to senior personnel and/or management for guidance and resolution Competencies: Ability to establish working relationships and effective communication with professional personnel of various backgrounds Ability to communicate clearly, both verbally and in writing Previous experience in writing factual, clear, and concise reports, to include situational analysis Ability to maintain daily scheduled activities and effective time management skills Excellent organizational and accuracy skills Previous involvement in coordination of medical services Ability to conduct thorough medical records review Ability to backss medical status and formulate recommendations Decisive and efficient decision-making skills Computer skills, to include proficiency with Microsoft Office Suite (Word, Excel, Power Point, Outlook) Education and/or Experience: (includes certificate & licenses) Associate's Degree or higher is required.
Preferred in medical or nursing field. Five (5) or more years previous case management experience Average Pay Rate: $50-55/hr Pay Range: Pay Range $49,400.00 - $89,300.00 The Leidos pay range for this job level is a general guideline only and not a guarantee of compensation or salary.
